given food on the blood-glucose level. The lower the GI
rating, the slower the absorption and infusion of sugars
into the bloodstream and hence a more gradual and healthier
digestion process.

Three: Unlike honey, table sugar lacks minerals and
vitamins (hence it's been often called empty calories),
they draw upon the body's nutrients to be metabolized into
the system. When these nutrients are all used up,
metabolizing of undesirable cholesterol and fatty acid is
impeded, contributing to higher cholesterol and promoting
obesity due to higher fatty acid on the organs and tissues.
That is why it is not uncommon for fat people to suffer
from malnutrition and many other health related problems.
So the message is, honey vs sugar, if you are watching your
weight, honey will be a smarter choice than sugar. Besides
the differences in nutrition, I feel sugar can never

Shoulder pain is a common affliction that affects many
people on a daily basis. Depending on the severity of the
pain this type of problem can have a significant, negative
impact on performing your occupation and your activities of
daily living. There are several ways that shoulder pain
can develop and I will discuss those below.

The most recognized way for pain to develop in the shoulder
is through trauma. This could consist of something obvious
like falling onto your outstretched arm causing your
shoulder to jam or trying to push or pull on a stuck
object. A less obvious traumatic event could be
forcefully using the muscles of the shoulder over time and
then pain occurs. This could happen during activities such
as chopping firewood, hammering, or exercising in a
repetitive fashion. The first traumatic event is
identified as a macrotrauma meaning it's a large and
obvious event. The second situation is termed a
microtrauma meaning it's less obvious and occurred with
repetitive insults to the muscle tissue. One does not
necessarily occur more than the other, but both can cause
significant injury and pain.

The macrotraumatic event can lead to injury of the muscle
by causing a major tear in the muscle fibers. A muscle
tear is exactly what it sounds like. The muscle fibers
actually rip apart and become separated from each other or
where the muscle tendon inserts on a bone in the shoulder.
You can have different size tears and these are usually
termed partial, full or full thickness tears. It is
important to remember that just because a muscle tear is
present that doesn't always mean you will have to have
surgery. Signs that a shoulder muscle is torn include but
are not limited to pain, weakness, and loss of motion.

A microtraumatic event or repetitive overuse can lead to
injury of the muscle by causing minor tears in the muscle
fibers. These are small in degree but enough to cause an
inflammatory process to occur which leads to pain. This is
a condition that we would commonly term as tendonitis (an
inflammation of the muscle or tendon). It is not the same
as a partial or full tear. The muscle fibers do not
actually separate from each other. This type of event can
cause significant pain and dysfunction. Signs that a
shoulder muscle is inflamed include but are not limited to
pain, weakness, and loss of motion.

Another common cause of shoulder pain is bursitis. A bursa
is a fluid filled sack that sits between muscle and bone.
The bursa is designed to protect the muscle from scraping
against the bone, thus it protects it from becoming
irritated against the bone. The bursa has a lot of nerve
endings in it. If the bursa becomes inflamed we term this
bursitis and it can be very painful. A bursitis can occur
in the shoulder as the result of a macro or microtraumatic
event as described above just like a muscle tear or
tendonitis can develop. Signs that a shoulder bursa is
inflamed include but are not limited to pain, weakness, and
loss of motion.

These are three of the most common pathologies that cause
pain in the shoulder. You will notice that the symptoms
that you may experience as a result of these conditions are
very similar. So how do you tell which problem you may
have? Well, to determine that you should consult a
competent Physical Therapist in your area that specializes
in the evaluation and treatment of musculoskeletal and/or
orthopedic pathologies. They will be able to successfully
diagnose the source of your pain and treat the problem
appropriately.

Death By Travel? The Little Known Risks of DVT and Tips for Avoidance
Lately air travel has become more and more unreliable and
annoying due to multiple flight cancellations and travel
interruptions. Any card carrying road warrior frequent
flier can relate multiple stories of being stuck almost
anywhere due to weather, mechanical issues, personnel
problems and lately...the abrupt FAA grounding of all MD-80
aircraft. Frank's story went a little differently, though.
Frank is a road warrior in every sense of the word: he
flies often and at least monthly finds himself in another
country. When he's not flying, he's driving to another
sales call. Frank is the Vice President of a manufacturing
firm and is responsible for making all his customers
happy...something that still requires face time! Frank was
having a typical week, three cities in 5 days, when he woke
up with a red, hot swollen left leg and shortness of
breath. Frank's travels had been interrupted by the dreaded
traveler's blood clot, a Deep Vein Thrombosis, or DVT. Now
he wasn't fighting for the sales contracts, he was fighting
for his very life! This may have been avoided if Frank had
learned his risk factors and followed some simple travel
tips.

A DVT (Deep Venous Thrombosis) is a condition in which a
blood clot or blockage forms in the deep veins in the leg.
These can develop in other places in your body, but are
much more common in the lower legs. If the clot breaks free
and travels through the bloodstream, it can actually lodge
in the lung. This clot in the lung is called a PE
(pulmonary embolism), and can cause shortness of breath,
difficulty breathing and even death!

How common is DVT? Each year, 600,000 patients will
experience some kind of venous thromboembolism or DVT. Each
year, at least 50,000 and perhaps as many as 200,000
patients will die from blood clots that obstruct blood flow
to their lungs (pulmonary embolism). It is amazing that the
complications from DVT have been reported to kill up to
200,000 people a year, more than AIDS and breast cancer
combined, but no one seems to care much about it. The
airline and travel industry has given it lip service after
a few well-publicized deaths; but the general traveling
public awareness of this potentially fatal public health
concern from travel is relatively low. Who is at risk for
DVT? There are certain groups of people who are more at
risk of developing these life-threatening clots. Risk
factors include varicose veins, blood clotting disorders,
recent childbirth or pregnancy, use of birth control pills
or hormone replacement therapy, obesity, and heart disease.
The risk is even higher in patients over 40, those with
casts immobilizing their foot and ankle, a sedentary
lifestyle, smoking history, certain types of cancer,
chronic illnesses like lung disease or inflammatory bowel
disease (Crohn's or ulcerative colitis) and recent major
surgery. This is further compounded by sitting for long
periods of time in a car, airplane or train, especially
with your legs crossed. It seems like almost everyone
traveling this summer has one or more of these risk factors!

What are the symptoms of DVT? Many people have either no
warning signs or very vague symptoms prior to forming a
significant clot. The symptoms are usually: swelling in the
leg and calf, redness and increased warmth in your leg, and
pain in the inner thigh and calf. If you experience any
warning signs, immediately contact your doctor for an
evaluation. Do not mess with this potentially deadly
condition!

How is a DVT diagnosed? A simple ultrasound of the veins of
your leg can determine if you are suffering from a DVT.
Often, superficial thrombophlebitis can have the same
symptoms and is much less serious; so a timely ultrasound
evaluation is critical. Patients who receive early
treatment may reduce their incidence of pulmonary embolism
to less than 1%.

If you have risk factors for DVT and plan on taking a long
trip this season, follow these tips to reduce the
likelihood of developing a blood clot:

1. Do not travel for 4 to 6 weeks after major surgery
unless your life depends on it.
2. Exercise legs every 2 to 3 hours to get the blood
flowing back to the heart. Walk up and down the aisle of a
plane or train, raise and lower your heels while keeping
your toes on the floor, rotate ankles while sitting, and
take regular breaks on road trips.
3. Stay hydrated by drinking plenty of fluids; avoid
caffeine and alcohol.
4. Consider wearing compression stockings. These are
helpful in compressing the veins and decreasing swelling in
the legs and calf.
5. Talk to your doctor about whether aspirin or another
blood thinner is indicated due to your risk factors.
6. Exercise regularly, maintain a healthy weight and stop
smoking!

Remember, a DVT can put a real cramp in your vacation or
travel plans, and even potentially kill you! If you are at
risk, take precautions before you suffer from this possible
"killer legs".

don't have social support.

Most men try to do everything on their own, but weight loss
is easier if you have social support. On the other hand,
most women are more willing to recruit help from friends
and family when on a weight loss program.

Men and women can get social support from friends at work,
from their family, from people at the gym, or even in an
online fat loss forum.

#5) Men and women struggle with weight loss because of poor
execise choices.

Advantage: Tie.

Most guys and gals just show up at the gym without a plan -
or they just go for a jog and do some crunches. But without
a professional plan, both men and women will not lose
weight, but only waste time.

both men and women should follow a program containing the

The No Packing Rhinoplasty

The No Packing Rhinoplasty
One of the greatest fears of many patients who are
considering rhinoplasty surgery is that their nose will be
packed during surgery, subsequently requiring removal
sometime after surgery.

That thought is one of significant uncomfortability at best
and outright pain at the worst. I have found that many
potential rhinoplasty patients often ask....are you going
to pack my nose? After hearing this concern numeruos times
over the years, I have come to appreciate a patient's
apprehensions over the issue of packing in the nose after
rhinoplasty surgery.

The good news is that the need for packing a rhinoplasty
surgery patient is very infrequent. If you are having a
cosmetic rhinoplasty, where only the external nose is being
operated on (hence the concept of a cosmetic rhinoplasty),
then there is absolutely no reason to have to pack the nose
after surgery. If you having a 'rhinoplasty' for breathing
purposes only, otherwise known as functional nasal airway
surgery or septoplasty and/or turbinate surgery, then the
need for nasal packing exists but it is still in my
practice a very low likelihood.

The purpose of nasal packing is two-fold; 1) to control or
stop bleeding and 2) to help adapt the lining of the nose
back in its place against the cartilage or bone so no
bleeding occurs underneath it. Whe performing septal
straightening, a very important component of airway
surgery, you must first lift the lining off of both sides
before straightening it. Once straightened, the lining must
be put back in its original place. Historically, packing
the nose with gauze squeezed this lining back in place. The
use of packing has been replaced by many surgeons with
sewing it back into place through a sewing technique known
as quilting. This has virtually replaced the need for
packing, much to the applause of patients and plastic
surgeons alike. Only in cases where there is too much
bleeding at the end of surgery will packing be used to
control it, much like the way it is used to stop difficult
nose bleeds.

So to those patients considering most forms of rhinoplasty
surgery, this is one issue that you need not overly concern
yourself with! Modern rhinoplasty techniques have virtually
eliminated the need for this uncomfortable consideration.
Not only is the concern about pain associated with packing
removal eliminated, but the fear of not being able to
breathe through your nose at all ( a claustrophobic
sensation) right after surgery is also alleviated.
